Newgate soon learns that the local doctors are not really who they say they are. It turns out that all the staff with real doctors are imprisoned in the basement, and the self-management of the hospital is carried out by the mentally ill former military doctor Lamb and the maniac killer Finn, who rebelled and seized power in the clinic. The real prisoner director Benjamin S alt asks Edward to escape and, having told everything to the authorities, save them, or find the keys and free the prisoners, because time is running out - death will find them due to cold and disease. Newgate offersElise runs away with him, however, she refuses.

Despite his suspicions, Lamb tries to gain the trust of a new employee. After bringing him to the wing for especially violent patients, Silas plans to destroy the instrument of torture, but first he orders Edward to tie the hands of one physically strong patient "Ogre". Newgate copes by earning favor from the patient by calling him by his real name Arthur.

One day, Edward overhears a conversation between Lamb and Finn, who are worried that the new doctor will soon guess everything, but they are reassured by the fact that the imprisoned doctors are unlikely to live long. The conversation is interrupted by anxiety - two real employees were able to escape from the hospital. They are caught up, but one manages to jump off a cliff, and the other is killed on the spot. Despite Lamb's attempt to cover up the doctor's death with a fall from a horse, Edward exposes the deception. The resulting quarrel provokes a violation of mental balance in the fake director.

Newgate learns that Lamb is the clinic's most "problematic" patient, on whom the bullying method, which aims to find and destroy the patient's existing fears, did not work. Silas comes to the real director to talk about the benefits of a no-torture method in which all patients live a normal life without drugs. S alt responds to information about the effective method with skepticism, after which Lamb decides to get rid of the real director by tying him to a table and giving him a session of electroshock therapy. Edward takes part and witnesses the dire aftermath of the disappearanceDr.'s personality.

Newgate tries to poison the invader patients, but is caught and sentenced to the punishment of erasing his memory with electricity. Before the procedure, he asks Lamb to pull out a photo of Eliza from him in order to admire the girl for the last time, but instead, the fake doctor finds a photo of one of his victims, after which he delves into memories.

In the process of freeing Edward, Finn gets an electric charge, after ignition of which the hospital catches fire. Newgate finds Lamb and rescues him. At the end of the film, the young man leaves England with Eliza, turning out to be far from being who he claims to be.
